Plaintiff suffered fatal injuries when a tank exploded as he instructed to perform pneumatic testing as part of the process for repairing a leak at the bottom of a tank. The pneumatic testing was unsafe and ultimately caused the explosion, which resulted in one man’s death and another man being seriously injured.
